Although it is not easy to tell from the photo, you seem to just need some volume in the cheeks. This is pretty common as people age. You can do it with an implant, but it may be easier and less expensive to fill the volume with injectable fillers like Radiesse or Artefill. The other advantage to the injectables is that they are quite precise so the chances of looking overdone and unnatural are less.

THE PHOTO IS FAR FROM THE BEST, BUT IT IS STILL OBVIOUS YOU NEED FAT GRAFTING TO THE CHEEKS. THIS SHOULD REJUVENATE AND LIFT YOUR FACE. IN EXPERIENCED HANDS THE RESULTS ARE PERMANENT.

You have fat loss from your cheeks (and face), which happens normally. The best option is either fat transfer (where fat is taken from your belly or thighs) or synthetic filler injections. This would result in natural youthful appearance.

As we age we have decent and volume loss. In your case replace volume first before elevating by surgery. Fat grafting my choice.
