I am terribly sorry that you have gone through such an ordeal only to be left with your current result. Please take heart; your current problem with your breasts are entirely predictable based on your detailed description of your surgeries and your photographs, and your problem is also completely correctable.
You have a relative excess of skin and breast tissue that, as you said, is hanging off of the implant. Well actually, it is hanging off of the muscle that your implant is covered by. There may also be residual scar capsule on the outer surface of the muscle that is impinging and preventing the muscle from fully expanding. Regardless, the solution is a formal breast lift or mastopexy. This will redrape the breast tissue and skin over the implants and give you a normal contour to the breast and repostion and project the nipple areolar complex.
A history and physical exam might also reveal other issues that could also be addressed at the time of surgery, including removal of residual capsular scar tissue from the old pockets, and possible adjustment of your current implants. It looks like you have more natural breast volume than many women who have uneventful breast augmentations, so fat grafting won't be necessary.
This surgery should only be performed by a specialist in breast revision surgery and be certified by The American Board of Plastic Surgery.
