First and foremost, I have no experience in thigh and buttock lifts nor do I have experience in tummy tuck revision. I am a urogynecologist and aesthetic vaginal surgeon. However, I deal with pelvic pain, surgical complications, dysparunia (pain with intercourse) on a daily basis. I will give my opinion without having seen you or examined you so take what I say with a grain of salt. However, it may help narrow your search for solutions to your problem.
My thinking is that the current pulling upwards of tissue both from behind and the past pulling up on the mons pubis with the abdominoplasty from the past has resulted in a tightening of your vulvar tissues, like having a vice-grip on two sides and pulling on the middle. This may cause pain on both entry and with deep thrusting. You may need a band release that should be done by an experienced vaginal surgeon. This is where I would start if you have already exhausted the options given by your plastic surgeons. I am guessing they have discussed loosening of the skin or scar from you multiple surgeries. I am guessing they have recommended use of vaginal estrogens perhaps if you are in the menopausal or peri-menopausal years. Lastly, I am guessing they may have already suggested using progressively enlarging dilators to help stretch the vulvovaginal tissues without having to undergo surgery. These are some issues to consider.
