Is a skin graft necessary?? Will this heal on it's own?

I had a breast lift on 7/20/2016. I am 5 week post op. My left breast had lipo for a small reduction to match the right. The left breast was extremely swelled with considerable bruising that is still not completely gone around the center of my right breast. By week 2 post op I had a open wound at the t-spot under the nipple. This open spot has never healed. The initial treatment was 1x week debridement for 2 weeks with moist dressing. This was not successful. At 3 weeks my Dr. was leaving the country for 2 weeks. I sought treatment from a wound care specialist. That was 8 days ago. He treated it by debridement. At home I was to clean and place a piece of collagen on the wound every other day. The first week after this treatment and there was zero change. In fact the bottom half of the wound has ceased producing any of the white stuff you see on the top part of the wound. The bottom part of the wound is now a yellow color. This is week 5, my treatment is the same, collagen every other day. The wound care specialist stated we would probably need to start considering skin grafts. This has been devastating to hear.
I am a healthy 47 year old. I do not smoke, I eat healthy. I have had a previous tummy tuck and 2 c-sections with ZERO issues. How is it possible for a wound to "just not heal"
I am seeking additional advice as everything I am reading about skin grafts is that it is a last resort. This has greatly affected my life and am struggling to understand and cope with it all. I am desperate for help so any feedback would be greatly appreciated.
